When you call and make an appointment, you will be offered to be mailed, e-mailed or faxed an application for an evaluation. If accepted, you will be scheduled for an evaluation appointment, which does not establish a doctor-patient relationship.

If you are accepted for an evaluation, an appointment will be scheduled and a non-refundable $200 deposit must be received before that date (Payable by check, cash, MC/Visa/Discover/American Express). The deposit will be credited toward the $600 fee for the evaluation. The deposit is sacrificed for failure to show up for your appointment.

After the evaluation you may be offered a treatment plan or referred to another professional for your care.

Please send, fax or bring with you any medical records including recent lab work, imaging, etc. so Dr. Goldstein may review your records prior to your evaluation. Also, please write a personal narrative of your condition (or chief complaint), including a detailed account of the condition from the time it was first noticed to the present, a time line of all doctors consulted, effectiveness of previous treatments, any observations that other caregivers may have deemed irrelevant, any other feelings that you believe Dr. Goldstein should know for a complete picture.

Dr. Goldstein may request lab tests be performed at any facility patient chooses. Follow-up visits will be scheduled as necessary. Fees will vary depending upon treatment methods, and will be fully discussed in advance.

If you need documentation to make insurance claims for prescription medicine, lab tests, etc. we will be pleased to provide whatever you need. Although basic billing statements are provided at no extra charge, lengthy reports, forms or phone conversations with your claim processors may be charged at a rate that will be discussed with you in advance.

Because the time slot is specifically set aside for you, please give 24 hours notice if you must cancel for any reason, or the deposit will be forfeit.
