Dr. Goldstein does not accept any insurance, government programs, liens or third-party payments. Payment may be made by cash, check or Visa, MasterCard, Discover and American Express debit/credit card. Prospective new patients must submit a non-refundable deposit of $200.00 in advance with their evaluation application (which will be applied to the $600.00 evaluation fee). The initial evaluation with Dr. Goldstein does not establish a doctor-patient relationship. After the initial evaluation, Dr. Goldstein will discuss treatment options, which may include a treatment plan under the supervision of Dr. Goldstein, or a referral to another competent professional.
